We report a case of fatal pulmonary embolus in a consecutive series of 625 anterior cruciate ligament reconstructions. An analysis of the predisposing factors is presented as well as a literature review of thromboprophylaxis in anterior cruciate ligament reconstruction. To our knowledge, this is the first report of fatal pulmonary embolus after anterior cruciate ligament reconstruction.
